Send this completed form and all required proof of purchase to: ACDelco Consumer Rebate Headquarters, Attention: $20 ACDelco Fall Consumer Rebate, PO Box 3730, Highland Park, MI 48203. For further assistance, please call 1-888-899-1174. Limit one $20 Rebate per person.
Official 2008 Fall Consumer Mail-In Rebate Rules:
- To be eligible, the consumer must purchase a minimum of $80 worth of ACDelco Automotive Replacement Parts on one repair order and have the parts installed at a participating Independent Service Center between October 1, 2008, and October 31, 2008, (“the promotion period”).
- The consumer must submit the following items to receive their rebate check:
- Original dated repair order with business identification verifying the purchase of the qualifying products and their ACDelco part numbers from a participating Independent Service Center during the promotion period. Offer not valid at a new vehicle dealer or on products being replaced under warranty.
- The ACDelco official Mail-In Rebate form filled out completely and legibly.
- To receive a rebate in the amount of $20, the consumer must mail the completed Mail-In Rebate form, and the original repair order to the address indicated. Request must be postmarked by November 30, 2008, 2008. ACDelco is not responsible for late, lost, or misdirected mail, and all proofs of purchase become the property of ACDelco and will not be returned. The consumer should retain copies of documentation and proof of postmark for their files.
- Offer good only in the U.S. at participating locations. The offer is void where prohibited, taxed, or otherwise restricted. Limit of one $20 rebate per person. Cannot be combined with other offers. PLEASE ALLOW 6-8 WEEKS FOR PROCESSING FROM RECEIPT OF REBATE FORM, AND ALL REQUIRED PROOFS OF PURCHASE.
- The 2008 Fall Consumer Mail-In Rebate check is valid for 90 days from the date of issue.
- Failure to comply with the official Mail-In Rebate Rules will make your submission invalid and could delay or prevent the mailing of a rebate check.
